Vitality Hospice & Palliative Care is expanding and excited to introduce a newly created Full-Time Licensed Practical Nurse (LPN) position to support the growing needs of our patients and community.
This role provides the opportunity to deliver meaningful one-on‑one hospice care to patients in their homes or care facilities while working within a supportive interdisciplinary team focused on comfort, dignity, and quality of life.
Position DetailsFull‑Time Position
$35–$40 per hour (based on experience)
Newly created role due to organizational growth
Structured weekday schedule
Field‑based role with local travel
Mileage reimbursement provided
Work setting: patient residences and care facilities
Flexibility when available
Under the direction of the RN Case Manager and clinical leadership, the LPN will:
Provide skilled hospice nursing care in accordance with each patient’s individualized plan of care
Perform nursing assessments and promptly report changes in patient condition
Support pain and symptom management to promote comfort and quality of life
Assist patients with activities of daily living (ADLs) as needed
Educate patients, families, and caregivers on care needs and self‑care strategies
Accurately document patient visits and care provided in a timely manner
Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care
Participate in on‑call rotation as needed
Active and unencumbered LPN license in the state of practice
Minimum 1 year of nursing experience preferred (hospice experience a plus)
Understanding of hospice philosophy and care for terminally ill patients
Strong assessment, communication, and documentation skills
Ability to work independently and manage time effectively
Valid driver’s license, reliable transportation, and current auto insurance
