Vitality Hospice & Palliative Care is expanding and excited to introduce a newly created Full-Time Licensed Practical Nurse (LPN) position to support the growing needs of our patients and community.

This role provides the opportunity to deliver meaningful one-on‑one hospice care to patients in their homes or care facilities while working within a supportive interdisciplinary team focused on comfort, dignity, and quality of life.

• Full‑Time Position

• $35–$40 per hour (based on experience)

• Newly created role due to organizational growth

• Structured weekday schedule

• Field‑based role with local travel

• Mileage reimbursement provided

• Work setting: patient residences and care facilities

• Flexibility when available

Under the direction of the RN Case Manager and clinical leadership, the LPN will:

• Provide skilled hospice nursing care in accordance with each patient’s individualized plan of care

• Perform nursing assessments and promptly report changes in patient condition

• Support pain and symptom management to promote comfort and quality of life

• Assist patients with activities of daily living (ADLs) as needed

• Educate patients, families, and caregivers on care needs and self‑care strategies

• Accurately document patient visits and care provided in a timely manner

• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care

• Participate in on‑call rotation as needed

• Active and unencumbered LPN license in the state of practice

• Minimum 1 year of nursing experience preferred (hospice experience a plus)

• Understanding of hospice philosophy and care for terminally ill patients

• Strong assessment, communication, and documentation skills

• Ability to work independently and manage time effectively

• Valid driver’s license, reliable transportation, and current auto insurance
