- Advanced Practice Nurse - Southwest NJ Team
PRN
Responsibilities- Conduct re-certification visits to patients on hospice in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
- Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, ongoing aggressive treatment).
- Consult with the referring physician, the acute care team, and other members of the interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of the IDT.
Master's Degree in Nursing
Professional Experience- Must have a Nurse Practitioner's license
- Must have 2-4 years experience working within hospice or related field
- Must have a Master's Degree
- Must have a valid driver's license and a reliable source of transportation
- Must be willing to travel
