We are currently seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our interdisciplinary team. The LCSW will provide emotional support, counseling, and advocacy for residents and their families, ensuring their psychosocial needs are met. This role is essential in promoting residents' well-being and enhancing their quality of life within the facility.

• Conduct psychosocial assessments and develop individualized care plans.
• Provide counseling services to residents and families.
• Collaborate with medical and nursing staff to support residents’ mental and emotional health.
• Assist with discharge planning and coordination of community resources.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Participate in care plan meetings and interdisciplinary team discussions.

• Certified or Licensed Social Worker required
• Masters degree preferred but not required
• Minimum of 1 year of experience working in a nursing facility or long-term care setting.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a collaborative team.
