Job Summary

The Care Nurse Supervisor (CNS) manages the successful operation of the Resident Care Department under the leadership of the Resident Care Director (RCD). The CNS is responsible for the oversight, coaching, and direct supervision of all resident care service providers in the community.

Responsibilities
  • Demonstrates expertise in clinical skills and nursing judgment within their scope of practice.
  • Identifies and assesses the clinical status of potential residents.
  • Obtains the medical information release form from the primary care physician as needed.
  • Develops a plan of care in collaboration with the RCD based on identified physical and psychological problems, sets resident-specific goals and interventions, and successfully implements the plan.
  • Creates a service plan in collaboration with the RCD, adhering to state regulations.
  • Coordinates care needs with community providers via an effective case‑management process.
  • Measures and accurately records vital signs (temperature, pulse, respiration, blood pressure, weight, and height) as appropriate per state regulations.
  • Oversees ancillary health services as needed.
  • Provides ongoing supervision of residents’ health‑related activities, follows healthcare instructions, and promptly seeks and obtains medical attention when necessary.
  • Recognizes and assists in preventing elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
  • Conducts SAMM/MAR and RCA documentation audits bi‑monthly.
  • Conducts assessments and reassessments per Benchmark standards.
  • Assesses risk proactively.
  • Facilitates weekly resident‑tracking meetings in the absence of the RCD.
  • Ensures resident information is communicated to staff through a report process and calendar (e.g., appointments, outings).
  • Communicates and documents residents’ material needs (e.g., briefs, clothing, toothpaste, pads).
  • Notifies the RCD of incidents in a timely manner and accurately completes incident reports before the end of the working day.
  • Observes and reports significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director as required by state regulations.
  • Responds to emergencies and personal emergency response system calls, knowing when to call for backup.
Compliance and Safety
  • Ensures compliance with all resident‑care‑related regulations (e.g., medication management).
  • Demonstrates infection prevention and control.
  • Demonstrates personal and resident safety.
  • Reviews and demonstrates fire, safety, and elopement procedures.
  • Ensures a follow‑up visit occurs after an incident, accident, or injury.
  • Ensures physician and family notification per state regulations.
  • Actively participates in the QA process for assignments.
Direct Supervision
  • Directs and supervises the daily work assignments of Resident Care Assistants (RCAs); serves as the first point of contact for RCAs regarding assignment questions or concerns, and provides input into performance reviews and disciplinary actions.
  • Participates in hiring and termination processes related to resident‑care associates, including sourcing, screening, and facilitating terminations.
  • Conducts or supervises scheduling for all RCA staff.
  • Mentors, guides, and directs Lead Resident Care Assistants in all aspects of their job.
  • Participates in training and in‑services as required by Benchmark and state regulations.
Qualifications
  • Graduate of an approved LPN program (per state requirements).
  • Holder of a valid, unrestricted nursing license.
  • Minimum 1–2 years of nursing experience.
  • Previous experience with the elderly in paid or volunteer positions highly desirable.
Required Skills & Abilities
  • Demonstrates strong supervision and management skills.
  • Understands and can develop Resident Care Service Plans.
  • Understands advanced directives and end‑of‑life care.
  • Possesses knowledge of assisted‑living philosophy.
  • Understands nursing function in assisted‑living settings.
  • Understands quality‑of‑care and quality‑of‑life objectives.
  • Possesses current CPR, First Aid, and Heimlich certifications, if required.
  • Possesses working knowledge of state regulations.
  • Keeps knowledge of policies in the resident handbook.
  • Excellent verbal communication skills; able to relate positively and professionally to a diverse resident and employee population.
  • Flexible and works well in a team‑collaborative environment.
  • Maintains highest level of confidentiality regarding residents, staff, and community.
  • Demonstrated ability to set priorities and complete assignments in a timely manner while managing multiple duties and responsibilities.
  • Proficient in documenting in a resident electronic health record (EHR) and using MS Office.
  • Proficient in using state audit compliance tools.
