The OpportunityECC Adult Social Care is making a transformational shift from a focus on long-term care support, to a model with four key principles: prevention, early intervention, enablement and safeguarding. Enabling people, to access information and tools, to live independently and allowing a positive response to growing service demand in a sustainable way, within the financial constraints of ECC.
The Integrated Neighbourhood model of practice is ever evolving and developing. Working within one of the Mid Locality Teams you will be an influencer of change and progression. The ethos being to support adults' prevention to hospital and with discharge from hospital, when safe and appropriate to do so, and continuing their care and assessment out of hospital.
Wherever possible Adults will be supported to return to their home for assessment. Your support will be focussed on the Teams within the INT sites enabling adults to remain as independent and self-caring as possible in their own homes. Some Adults may have been discharged to a temporary care home environment to aid their recovery and for a period of assessment to be able to identify their short- or long-term care and support needs.

Adult Social Care
- Interaction with INT and Acute partners to raise TEC Awareness and deliver/support with training to increase trusted assessors.
- Increase engagement within the acute and community hospitals and identify Adults who would benefit from TEC.
- Work alongside colleagues in the locality to review cases to prevent admission and before discharge to consider TEC.
- To attend Digital Champion and Ops meetings on behalf of the locality.
- Link regularly with TEC team and Livity Life regarding choices of TEC and OT Assessment requests, queries, and opportunities.
- Obtain case studies and share good practise.
- Share learning and knowledge with hospital and community colleagues to create awareness of the TEC offer.
- Identify opportunities to embed TEC into pathway across the locality.
- Identify opportunities to test and learn new technology and evaluate effectiveness.
- Liaise with Livity Life (Millbrook) around hosting regular TEC drop-in sessions to display equipment.
- Provide quarterly feedback for the BCF boards on progress of role.
- Experience with in a social care setting, an NVQ/BTEC Level 3 or equivalent is desirable.
- Evidence of continuing professional development and knowledge in relevant professional area.
- Experience of building strong, effective relationships with adults, their families and carers in times of challenge and crisis.
- Experience of undertaking direct, activity-based work with adults.
- Knowledge of legislative and policy guidance applicable within Social Care.
- Proven track record of building working relationships with partners and external agencies.
- A certificate in First Aid and/or knowledge of Sign Language is desirable in this role.
