Patient Flow Co‑ordinator

Key responsibilities include:

  • Providing advice and patient management relating to complex discharge scenarios.
  • Working collaboratively with the multidisciplinary team to facilitate appropriate and timely discharges by providing continuity in discharge planning and effective dissemination of information.
  • Ensuring all relevant social service referrals are made in a timely manner, monitoring response times, and acting upon any delays.
  • Serving as a point of contact for the ward regarding discharge and ensuring the ward carries out the specific responsibilities outlined within this Job Description.
  • Being proactive with regard to patient flow and discharge.

Additional details: This post is a 6‑month fixed‑term contract. The role is not sponsored.

Main duties of the job

The Assistant Patient Flow Co‑ordinator role is a varied role with a primary focus on patient flow and patient discharge. You will be the point of contact on the ward for information and advice to staff, patients and relatives/visitors.

You will work collaboratively with the Multi‑Disciplinary Team (MDT) to facilitate safe and timely discharges by providing continuity in discharge planning and effective dissemination of information.

Day‑to‑day responsibilities include, but are not limited to, attending MDT board rounds and collating detailed information about each patient, escalating diagnostics and speciality reviews, patient transport bookings, communicating with patients and their families, liaising with pharmacy, ensuring there is a clear plan from all in the MDT and providing effective written documentation about discharge progress.

You will be computer literate; have excellent communication skills both written and verbal.

Job responsibilities

This will be a full‑time post working Monday to Friday shifts covering 08:00-16:00 & 10:00-18:00. You will be employed under the Flow Hub at Broomfield and will work within a team covering a group of ward within a zone. You may be required to cross cover as the service requires.

Experience
  • Previous experience working with computers
  • Previous NHS hospital experience within a ward setting
  • Experience in the care sector
