The Discharge To Assess Service focuses on enabling adults to transition smoothly from hospital to community, preventing readmissions and fostering a safe discharge process. By working closely with healthcare partners, the team assesses adults post‑discharge to ensure their recovery and independence continue to be prioritised in a supportive environment.
