Case Manager PRN

Job Summary

Parkridge Medical Center, part of HCA Healthcare, seeks a Case Manager PRN to coordinate care for patients and families throughout the entire care journey. The role involves collaborating with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care.

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team.
• Collaborate closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient‑centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.

• (RN) Registered Nurse
• Associate Degree
• 2+ years experience in case management OR 3+ years’ experience in clinical nursing.
• Inter Qual experience is very helpful.

Salary: $62,834.00 – $75,000.00.
