Job Summary
The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day‑shift role, covering 3‑10 hour shifts from 8:00 am to 7:00 pm. The provider will collaborate with physicians, nurses, and other healthcare professionals to deliver patient‑centered care in a community health setting.
Responsibilities- Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.
- Provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention.
- Manage follow‑up care, coordinate referrals to specialists, and ensure continuity of care for patients.
- Maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives.
- Communicate effectively with patients, collaborate with interdisciplinary teams, and foster trust within the community.
- Address social determinants of health by connecting patients to appropriate resources and support services.
- Coordinate care and support patients, families, and caregivers in self‑management and behavior change.
- Manage the practice’s patient population and participate in the practice’s quality improvement process and performance evaluation.
- RN with a master’s degree in nursing and certification in family and/or adult nurse practitioner; graduate of a nationally accredited school of nursing.
- Previous experience in a Federally Qualified Health Center (FQHC) is strongly preferred.
- Current licensure to practice nursing in the State of Tennessee.
- Eligibility to sit for certification in area of specialty.
- Certification in areas of specialty is preferred.
- The Tennessee nursing license will be a single state license for Tennessee residents or non‑compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non‑Tennessee residents.
- Ability to collect, document, and analyze patient data.
- Demonstrated knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient’s status.
- Experience in developing and implementing evidence‑based protocols, health screening programs for adolescents and adults, and identification protocols for high‑risk individuals.
- Ability to collaborate with the healthcare team in the development and implementation of testing and treatment according to protocols.
- Participation in development and implementation of community and corporate education on internal medicine topics.
