Overview

We are seeking a Medical Social Worker (MSW) for our Home Health team in Smyrna, TN, to provide emotional support, counseling, and resource coordination to patients.

• Assist patients and families with social, emotional, and financial concerns, promoting effective coping skills.
• Deliver social work services aligned with the care plan and communicate clinical changes to the healthcare team.
• Educate patients, families, and team members on interventions and treatment plans.
• Perform care planning, transfers, and discharge processes.
• Identify and address risks in the patient s environment to ensure safety and effective care.

• Master s degree in Social Work
• Current unencumbered license
• At least 1 year of healthcare social work experience
• CPR certification
• Valid driver’s license
• Reliable transportation
• Ability to commute within the area
