Clinical Pharmacist
Job Summary
A licensed professional pharmacist employed by Erlanger Medical Center responsible for providing pharmaceutical care for all patients. This includes reviewing patient orders and charts, interacting with healthcare providers and patients during physician rounds, and providing drug therapy and clinical services.
EducationRequired:
Must be licensed or eligible for licensure in Tennessee and meet the following criteria:
Doctorate in Pharmacy (Pharm
D) or advanced training (3‑5 years hospital experience, completion of a pharmacy practice residency, fellowship, and/or board certification). Preferred:
Completion of a pharmacy practice residency and/or BCPS certification.
Required:
Knowledge of general pharmacy practice and pharmaceutical care with emphasis on hospital practice; knowledge of organizational functions, policies and regulations; knowledge of current trends and new developments; ability to learn principles and practices applicable to those areas requiring special education.
Preferred: 3 years of appropriate clinical experience.
Certification & LicensureRequired:
Current and valid license to practice pharmacy in the State of Tennessee, or be eligible to be licensed as a pharmacist by reciprocation or examination.
Preferred:
Board certification.
- Provide pharmacy review of medication orders prior to administration.
- Work collaboratively with clinicians and health‑care providers to implement innovative disease management programs and clinical pharmacy services.
- Develop medication use management programs within Erlanger Health System including clinical guidelines, critical pathways, disease management, and drug use programs.
- Provide didactic and experiential training in clinical pharmacy for pharmacy students and residents.
- Participate in clinical research projects, presentations, and publications.
- Serve as adjunct faculty in accredited schools of pharmacy.
- Develop and maintain competency of all pharmacy staff.
- Assist in residency program development.
- Maintain knowledge of all legal requirements for drug storage, handling, and dispensing.
- Stay informed on new drugs and investigational drugs.
- Attend professional literature, meetings, and seminars.
- Develop new programs, services, practices, and educate department. >
- Focus on cost avoidance and savings.
- Facilitate proper information transfer among staff.
- Initiate pharmacy services as needed.
Provide medication use teaching for discharge medications.
Change work conditions based on volume and acuity level of patient population. Daily use of computer/order entry/answering phones.
