At Avenues Home Care, our caregivers are the front line and most common representative. As an In Home Private Duty Caregiver, you will care for the elderly, vulnerable, disabled, and those unable to care for themselves. You must keep the client’s needs above all else and perform the following duties with utmost care. Our office in Chattanooga serves clients in Chattanooga and surrounding areas.

• Assist the client with Activities of Daily Living (ADLs) as specified in the client’s plan of care created during intake. Activities may include bathing, dressing, grooming, personal hygiene, toileting, transferring, eating and continence care.
• Assist with medication assistance, housekeeping services, laundry, meal preparation, feeding, shopping, transportation to and from appointments and companionship.
• Perform other household tasks as specified in the client’s plan of care.
• Report daily activity to your supervisor through the agency management system and activity logs. Communicate via phone, email, agency app or face‑to‑face as needed.

• Practice safety in all activities concerning Avenues Home Care clients.
• Lift or push up to 50 pounds.
• Demonstrate honesty, accountability and integrity.
• Provide superior care to Avenues Home Care clients.
• Remain vigilant of any potential care concerns or changes in condition.
• Maintain personal appearance in compliance with Avenues Home Care Policies, including dress code.
• Report shifts or scheduling changes promptly.
• Report any suspected abuse or neglect immediately as required by law.
• Document any unusual behavior, events, conditions or people.
• Maintain confidentiality of records and correspondence.
• Attend meetings and in‑services as requested.
• Abide by all company policies and procedures.
• Ensure protection of confidential information per company policies and HIPAA regulations.
• Other duties as assigned.

• Good communication skills.

• Proficiency with the Homecare Company Management System – Clear Care.

• Maintain "Fit for Duty" physical condition, a valid driver’s license, automobile insurance and possess the following minimum requirements:
  • 18 Years of Age or Older
  • CNA License or alternate experience as a caregiver (preferred)
  • CPR & First Aid Certifications (preferred)
  • Reliable Transportation

Prior experience assisting with ADLs or caring for elderly, disabled, and other vulnerable clients.
