Care Coordinators must be organized, personable, and capable of balancing the needs of multiple patients as well as their healthcare providers and insurance representatives to address barriers that will ensure the patient can navigate their recovery experience. Skills include active listening, service orientation, social perceptiveness, critical thinking, time management, active learning, good judgment and decision making, verbal communication, and the ability to monitor progress and needed interventional actions.
Responsibilities- Act as an integral member of the ReVIDA Recovery multidisciplinary team by supporting patients in obtaining individualized goals, building and strengthening linkages with community resources and family members, and organizing patient care activities to achieve safer and more effective care.
- Assist individuals in opioid use disorder treatment in developing service plans, referrals, and linkage, ensuring access/continuity of care throughout the mental health and primary care system.
- Ensure patients have resources to acquire medication, transportation for appointments, and attendance at appointments.
- Facilitate access to psychosocial rehabilitation, support, employment, and housing options while encouraging use of community supports to manage substance use disorders.
- Work is sedentary and ambulant with occasional physical exertion; must be able to assist in emergencies and be mobile as required. Visual and hearing abilities adequate for communication and documentation are required.
- Must be able to relate to and work with mentally and physically ill, disabled, emotionally upset, and hostile patients; handle multiple situations with emotional stability and coping skills.
- Compliance with universal precautions due to risk of exposure to infections and blood borne pathogens. Subject to variable work schedules and shift changes.
Supervision and competency evaluations are provided through facility monitoring activities, direct observation, staff meetings, in-services, management meetings, individual meetings, Employee Improvement processes, reporting, and annual competency review.
Competencies- Coordinate care to address medical, behavioral health, and social determinants of health (e.g., housing, food insecurity, childcare) with documented referrals and tracking.
- Assist individuals in addressing barriers to referrals (e.g., transportation) and document interventions and outcomes.
- Communicate patient needs and preferences to the care team, ensuring practical and comprehensive care.
- Document and track referrals and outcomes; collaborate with the multidisciplinary treatment team (MDT) to address barriers and adjust plans accordingly.
- Utilize community-based support modalities and consider referrals to 12-step programs, peer recovery services, and other resources as appropriate.
- Organize and participate in interdisciplinary care planning meetings (monthly) and review the patient’s medical record and plan of care; update IPOC and ISP according to schedule.
- Develop an Individualized Plan of Care (IPOC) within 30 days of admission, using the DMAS template; update ISP/IPOC every 30/90 days as required.
- Collaborate with MDT members to create an Individualized Aftercare Plan and Discharge Plan; ensure alignment with DMAS ARTS/OBOT guidelines and maintain accurate patient documentation.
- Adhere to DMAS ARTS program requirements and OBOT guidelines; maintain attention to detail in written communication and patient documentation.
Care Coordinator Requirements:
- One year of previous experience working with SUD/OUD/BH populations required.
- Knowledge of substance use disorders, dual diagnoses, opioid use disorder, and recovery support services required.
- Tennessee: RN, LPN, MSW, BSW, or Equivalent.
- Virginia: RN, CSAC, CSAC-R, CSAC-A, BSW, or Equivalent (Must be registered with the Board of Virginia).
Knowledge of applicable State Code of Regulations and 42 CFR Part
2. Compliance with professional standards; adherence to NAADAC Code of Ethics and other applicable Codes of Ethics for the profession.
Participate in continuing education as required by immediate supervision; maintain current professional certifications as required by the state, in good standing.
