Cognitive assessment with a blend of primary care - Population:
Patient ages 55+ - Resource team includes an LCSW
- Eligible for quarterly bonuses following the guarantee period

- MD or DO degree
- Board certified/board eligible
- Ability to obtain an active medical license in the state of Tennessee and/or Georgia
