The Advanced Practice Provider (APP) will provide comprehensive clinical care for both the Hypertrophic Cardiomyopathy (HCM) and General Cardiology populations. In the HCM Program the APP see initial visits, follow up visits and mavacamten follow ups; as well as general cardiology services such as HTN, HLD, CAD. The APP will deliver evidence-based cardiovascular care tailored to the patient's needs, serve as a patient advocate, and function as a key member of a multidisciplinary team committed to excellence in patient outcomes, education, and compassionate care.
Shifts
Monday-Friday, primarily daytime hours; no weekends, holidays or call expected.
Preferred Qualifications- Prior clinical experience in cardiology, internal medicine, or cardiovascular subspecialties.
- Experience with outpatient management of cardiovascular conditions.
- Full-time, outpatient clinical role based primarily in UVA's Ambulatory Cardiology clinics at Fontaine.
- Day shifts, Monday through Friday.
- Option for four 10-hour days.
Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.
Administration- none
- Partner with pharmacists to optimize medication therapy and patient adherence.
- Work with social workers to address behavioral health and social needs.
- Collaborate with registered nurses and navigators for care coordination and follow-up.
- Coordinate care with imaging, heart failure, and electrophysiology subspecialists as appropriate.
- Participate in interdisciplinary case conferences and care planning meetings.
- Independently evaluate, diagnose, and manage new and follow-up patients with HCM
- Perform cardiovascular assessments; order and interpret diagnostic testing; develop and implement individualized treatment plans.
- Manage cardiovascular risk factors and chronic conditions including hypertension, dyslipidemia, obesity, diabetes, and menopause-related risk.
- Manage general cardiovascular conditions such as hypertension, coronary artery disease, heart failure, arrhythmias, and valvular disorders.
- Provide patient and family education regarding disease processes, treatment options, procedural care, and long-term self-management.
- Provide medication management and follow-up care aligned with REMS program for mavacamten
- Coordinate diagnostic testing, referrals, and follow-up care.
- Document clinical encounters accurately and timely in the electronic medical record (EPIC).
- Model patient-centered, evidence-based care aligned with HCM Program goals.
- Contribute to fostering a multidisciplinary, collaborative clinic culture.
- Support quality and program development through adherence to established care pathways and performance metrics.
- Act as a clinical resource and subject matter expert in HCM care.
Education:
Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.
Experience:
Department and unit specific.
Licensure:
License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies.
Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.
Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive);
Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills;
Attention to detail and ability to write legibly;
Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.
The starting base rate for this role is $96,907.20 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
