Position: Registered Nurse (RN) - Continuum Home Health Case Manager, Charlottesville/Albemarle

Responsibilities

As a Home Health Registered Nurse, you will provide hands‑on nursing care to patients with a wide range of diagnoses, leading an interdisciplinary home‑health care team in partnership with physicians and other care team members. You will build trusting relationships with patients and caregivers, assess patient needs, develop individualized care plans and implement evidence‑based interventions, monitoring patient progress and adjusting care to ensure exceptional outcomes.

UVA Continuum Home Health is a department of UVA Health. We have been providing care in the community for 30 years гид. Minimum Requirements
  • Education:

    Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.
  • Experience:

    1 year of Relevant Experience.
  • Licensure:
    Licensed to practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. American Heart Association (AHA) Health Care Provider BLS certification required.
Physical Demands

Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20 มี–50 lbs. May be exposed to chemicals, blood/body fluids and infectious disease.

The starting base rate for this role is $82,992.00 annually. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education.
