Overview
Within the Emergency Department to proactively identify potential patient admissions and patients with needs for discharge. Works with Bed Center staff to identify inappropriate patient status issues (inpatient versus observation). Resolves issues of incorrect status through discussions with ED and Admitting Physicians by obtaining additional information to support admission order or determine alternative level of care. Coordinates discharges home from the ED and facilitates transfers to external placement facilities.
Maintains expert knowledge of Interqual Level of Care criteria.
- Screen all ED patients to determine potential admissions and the need for a clinical or psychosocial intervention.
- Discuss all potential admissions with the appropriate ED physician.
- Coordinate all discharges that require clinical intervention.
- Develop and maintain care plans for active management of patients who frequently seek ED care (e.g., list of Primary Care Physicians accepting new patients, community resources for families managing frail/elderly) or who may be discharged/transferred from the ED.
- Start discharge planning on admitted patients.
- In addition to the above job responsibilities, other duties may be assigned.
- Education:
Graduate of an accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire. - Experience:
3 years of relevant experience. - Licensure:
Licensed to practice as a registered nurse in the Commonwealth of Virginia. - Certification:
Care Management certification within 3 years of hire.
Job requires standing for prolonged periods, frequently traveling, bending and stooping. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly; ability to lift, push, or pull 20–50 lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
Position CompensationRange: $39.10 – $51.94 per hour.
