Overview

Sentara Health Plans is hiring an Integrated Case Manager – Registered Nurse for Charlottesville, VA and surrounding areas.

Full‑time, permanent position (40 hours). Standard working hours 8am‑5pm EST, M‑F. Position requires in‑person assessments and remote telephonic assessments for the LTSS Waiver population. Candidate must reside in Charlottesville, VA.

Responsibilities

Perform casework duties and provide case management services to patients, families and designated caregivers. Develop, participate and monitor multidisciplinary collaboration of services where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including terminal illness.

Education
  • Degree in Health & Human Services or related fields:
  • Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation.
  • All degrees must be from accredited institutions listed in the U.S. Department of Education College Accreditation database. Degrees from outside U.S. will be reviewed individually.
Certification / Licensure

No certification or licensure required.

Experience
  • Long Term Care – 1 year;
    Health Plan – 1 year;
    Medicaid – 1 year (required).
  • At least one year of mental health experience preferred.
