Job Summary
The Housing Case Manager is responsible for providing housing stabilization services to help clients achieve and maintain permanent housing while promoting economic and social/emotional self-sufficiency. This role involves conducting assessments, creating individualized case plans, linking clients to community resources, conducting home visits, and managing a caseload of 15-20 clients. The Housing Case Manager will work to support clients in overcoming barriers to stable housing and improving their overall well-being.
Core Responsibilities
- Provide case management and service coordination to assist clients in achieving optimal health and housing stability through person-centered, trauma-informed services.
- Work closely with the Housing Manager and Behavioral Health Team to complete initial psychosocial assessments for all clients.
- Develop and implement individualized care plans in collaboration with clients, addressing areas such as daily living skills, money management, employment linkage, benefits establishment, and access to substance abuse and mental health care.
- Assist clients in securing safe, permanent housing, ensuring long-term housing stability.
- Provide crisis intervention as needed, offering immediate support during times of urgent need.
- Identify client service needs and address barriers to accessing necessary resources.
- Maintain compliance with organizational workflows, policies, and procedures to ensure effective program delivery and accreditation standards.
- Prepare presentations and conduct monthly outreach sessions to raise awareness and engage clients.
- Facilitate group meetings and develop an activities calendar to support the psychosocial well-being of clients.
- Ensure confidentiality and security of sensitive materials regarding client programs and participants.
- Perform additional responsibilities or tasks as needed to support the organization’s mission and goals.
Knowledge, Skills, and Abilities
- In-depth knowledge of HIV/AIDS, the social determinants of health, housing instability, and homelessness, including the assessment, development of goals, and intervention techniques.
- Awareness and sensitivity to cultural differences and diverse family dynamics, with the ability to implement effective interventions tailored to clients' needs.
- Proficiency in Microsoft 365 and other relevant software tools.
- Strong written and oral communication skills, including the ability to document case management notes clearly and accurately.
- Familiarity with community resources and support services in Charlotte-Mecklenburg and the surrounding areas including SC.
- Knowledge of various housing resources, including governmental and private sector programs, and their protocols and expectations.
Qualifications
- Bachelor's degree in social work minimum of 2 years of case management experience with chronically homeless adults, severe & persistent mental illness, and/or substance use disorder.
- Master's degree in social work (MSW) minimum of 1 years of case management experience with chronically homeless adults, severe & persistent mental illness, and/or substance use disorder.
- Valid driver's license and reliable transportation
- Ability to read and interpret documents such as leases, medical records, background checks, court documents, and written documents.
- Ability to write reports and develop presentations
- HUD CPD Part 5 Income Calculation Certificate within 30 days
- Experience using HMIS or obtain HMIS Certification within the first 30 days.
