Job Summary
We are seeking a dedicated Registered Nurse (RN) – Home Health to provide skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and state regulations. The RN will be responsible for patient assessments, care coordination, supervision of LPNs and HHAs, and maintaining accurate clinical documentation.

Experience with home health workflows and electronic visit verification systems is preferred (training provided).

Key Responsibilities

  • Conduct initial home health visits and ongoing patient reassessments
  • Initiate and manage plans of care under physician orders
  • Perform comprehensive patient assessments and monitor progress
  • Observe, assess, and document patient symptoms and responses to care
  • Educate patients and caregivers on disease processes, medications, and care plans
  • Coordinate patient services and ensure continuity of care
  • Supervise and provide onsite oversight to LPNs and Home Health Aides (HHAs)
  • Communicate changes in patient condition to physicians and interdisciplinary team members
  • Complete required documentation, including OASIS/485 and clinical records per policy
  • Participate in case conferences to ensure coordinated patient care
  • Discharge patients from skilled nursing services when criteria are met
  • Maintain compliance with agency policies, state regulations, and continuing education requirements

Qualifications & Requirements

  • Active Registered Nurse (RN) license in North Carolina
  • Minimum 1 year of RN experience
  • Associate’s Degree in Nursing (required)
  • High School Diploma or equivalent
  • Strong communication and interpersonal skills
  • Ability to work effectively with patients, families, and healthcare staff
  • Home health experience preferred
  • Wound care experience preferred
