- Assumes responsibility for the effective coordination of patient/caregiver care.
- Conduct nursing assessments to assist in the overall management of patient/caregiver needs.
- Make visits to provide appropriate interventions; follow established plan of care and evaluate progress toward goals, revising the plan as necessary.
- Develop, implement, and evaluate plans of care, incorporating therapeutic, preventive, and palliative nursing actions, and specifically addressing patient/caregiver education.
- Emergency triage by telephone and home visits.
- Provide interventions consistent with the plan of care and perform all required interventions for each visit; identify new problems or needs when they occur.
- Facilitate patient and caregiver independence as possible.
- Initiate and maintain liaison relationships with healthcare providers to coordinate multidisciplinary care; consult with patients, caregivers, physicians, facility staff and interdisciplinary team when initiating care or when significant changes occur; reliably communicate pertinent information.
- Identify and refer psychosocial needs to VIA Health Partners medical social worker as appropriate.
- Identify learning needs and provide necessary instruction.
- Demonstrate nursing care procedures and utilize teaching materials, tools, and written information appropriately.
- Encourage and allow time for questions and consultation.
- When providing hospice care, appropriately utilize VIA Health Partners policies, patient related protocols, standing orders and guidelines, including Enclara Pharmacia MUGs;
Medline, and Pro Hospice as applicable.
- Indirect patient/caregiver care responsibilities.
- Maintain productive and professional relations with external agencies and healthcare providers.
- Store supplies safely and keep work area clean, secure, and well‑maintained.
- Maintain appropriate utilization of resources to ensure cost‑effective delivery of healthcare.
- Apply infection control principles and ensure universal precautions per professional practice standards.
- Keep clinical records current by synchronizing laptop information throughout the shift.
- Complete and submit all clinical documentation per VIA Health Partners guidelines.
- Maintain current medication profile as directed by physician and document medication teaching.
- Record time/mileage accurately.
- Responsible for related duties as required and/or assigned.

- Ensure professional reputation is maintained and projected.
- Assume responsibility for professional development and stay abreast of current trends; incorporate new information and methods into practice.
- Maintain current professional licensure/certification (RN license, CPR certification, CEUs).
- Understand that everyone is a customer and strive for positive, productive relationships.
- Responsible for all material reviewed at nurses meetings, after hours meetings, and all staff meetings, whether in attendance or not.
- A diploma or associate’s degree in nursing is required; bachelor’s degree preferred.
- Active, unrestricted multistate RN license is required.
- Current CPR required; CHPN preferred.
- Minimum 2 years of related experience in direct care of patients with life‑limiting illnesses (hospice, palliative, family medicine, internal medicine, geriatrics, oncology, home health care, medical‑surgical).
- Prior experience working in an environment focused on pediatric population is required for Kids Path™ Nurse with BCLS (CPR) training for adult and child/infant required.
- Computer proficiency required.
