Essential Functions
  • Delivery of Patient Care
    • Assumes responsibility for coordination and delivery of patient/caregiver care.
    • Conducts nursing assessments to assist in overall management of patient/caregiver needs.
    • Develops, implements, evaluates, and updates the plan of care to ensure uninterrupted care and goal achievement.
    • Facilitates patient and caregiver independence to the extent possible.
    • Utilizes VIA Health Partners patient protocols, standing orders and guidelines when providing hospice care.
    • Conducts effective patient and caregiver education and enables questions and consultation.
    • Provides education to patients and families as needed.
    • Communicates effectively with members of the IDG, patients, caregivers and other health professionals to ensure continuity of care.
    • Maintains open lines of communication with supervisor and keeps supervisor apprised of any complex issues.
    • Applies principles of infection control and follows universal precautions guidelines.
  • Interdisciplinary Group
    • Attends, participates and may facilitate IDG and special care conferences.
    • Establishes and maintains effective working relationships with team members.
    • Identifies specific patient/caregiver needs and makes referrals to appropriate team members.
  • Documentation
    • Ensures accurate, complete, succinct and timely clinical documentation per VIA Health Partners guidelines and licensure/certification standards.
  • Professional and Personal Accountability
    • Demonstrates competency in knowledge and skills.
    • Maintains VIA Health Partners professional reputation.
    • Assumes responsibility for professional development and stays abreast of current trends.
    • Maintains current professional licensure, CPR certification and CEU requirements.
    • Maintains productive professional relations with external agencies and healthcare providers.
    • Demonstrates time‑management skills and ability to respond to crisis.
  • May perform other duties as required
  • (IPU) Cleans patient rooms following cleaning procedures, maintains facility cleanliness and uses appropriate protective equipment.
Qualifications
  • Diploma or associate degree in nursing (Bachelor’s degree preferred).
  • Current, active, unrestricted compact (multistate) license as a Registered Nurse (RN).
  • Current CPR required; CHPN preferred.
  • Minimum 1‑year related experience for inpatient units (IPU) and 2 years for other nursing positions; direct care of patients with life‑limiting illnesses required.
  • Experience may include hospice, palliative care, family medicine, internal medicine, geriatrics, oncology, home health care or medical‑surgical settings.
  • Computer proficiency required.
