The Clinical Nurse Coordinator serves as the clinical backbone of Care Ring Clinic, providing essential nursing expertise that elevates the quality and safety of patient care. This role requires an experienced, adaptable nurse comfortable working in a dynamic and evolving environment. The Coordinator focuses on clinical assessment, triage, patient education, preventive care services, and clinical care coordination for complex or high‑risk patients.
The Coordinator works collaboratively with the Medical Director, Nurse Practitioners, and medical assistant team to ensure comprehensive, patient‑centered care delivery, enhancing the clinic’s capacity to serve our 6,000‑patient community.
- Conduct telephone and in‑person triage of patient concerns using clinical judgment and evidence‑based protocols.
- Assess patient symptoms, provide clinical guidance, and determine appropriate level of care.
- Make independent nursing decisions regarding urgent vs. non‑urgent care needs.
- Communicate triage outcomes and recommendations to patients and document appropriately in the EHR.
- Escalate complex or emergent situations to providers as indicated.
- Provide clinical oversight to medical assistants regarding patient care concerns.
- Administer immunizations per CDC guidelines and clinic protocols for all age groups.
- Perform health screenings including blood pressure checks, glucose monitoring, and preventive health assessments.
- Conduct comprehensive nursing assessments for complex patients prior to provider visits.
- Perform wound care, including dressing changes and assessment of healing progress.
- Administer injectable medications as ordered by providers.
- Collect laboratory specimens, including blood draws when needed.
- Assist providers with in‑office procedures as needed.
- Provide clinical care coordination for patients with complex medical needs or multiple chronic conditions.
- Conduct post‑hospital discharge follow‑up calls to assess patient status and identify care needs.
- Coordinate care plans with specialists and other healthcare providers.
- Identify gaps in care and work with patients and providers to address barriers to treatment adherence.
- Support transitions of care between different care settings to improve outcomes and reduce readmissions.
- Monitor high‑risk patients proactively through outreach and follow‑up.
- Provide comprehensive patient education on chronic disease management including diabetes, hypertension.
- Teach patients about new diagnoses, treatment plans, medications, and self‑management strategies.
- Counsel patients on preventive health, nutrition, smoking cessation, and lifestyle modifications.
- Educate families and caregivers on supporting patient care at home.
- Develop and provide educational materials tailored to patient health literacy levels.

- Review medication lists with patients and identify potential issues with adherence or side effects.
