Join Curana Health as a Nurse Practitioner or Physician Assistant leveraging your expertise to deliver primary care in skilled nursing and senior living facilities across North Carolina. This is a travel‑based position that supports multiple facilities each week.

As a clinician, you will provide direct patient care, promote wellness, manage chronic conditions, and coordinate care with other providers. The role includes comprehensive assessments, patient education, documentation in electronic medical records, and participation in quality improvement initiatives.

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
• Collaborate with facility staff, family members, and healthcare providers to ensure coordinated care.
• Educate residents, families, and staff on disease prevention and wellness.
• Maintain accurate documentation within EMR and comply with regulatory standards.
• Participate in quality improvement initiatives to reduce hospital readmissions and improve patient satisfaction.

• Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
• Master’s Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem‑solving skills.
• Excellent written and oral communication skills.
• Organizational skills, punctuality, and ability to work collaboratively.

This role requires travel to and from one or more assigned facilities. Travel may be subject to additional licensing or insurance requirements; periodic motor vehicle record checks may be conducted as a condition of continued employment.
