Teammate will be PRN and will help over CHG nurses at Main, Mercy, University, Cabarrus, Union, and Pineville.

Essential Functions
  • Works with provider(s) to create focused patient-centric discharge plans, addressing barriers to discharge for patients with longer than expected stays.
  • Coordinates evaluation, testing, management, and follow-up of patients in collaboration with the attending physician and healthcare team.
  • Rounds with physicians and acts as a liaison between physicians, healthcare team, patient, and family members.
  • Promotes and assists with patient and family education supporting the treatment plan and educational needs for continuity of care.
  • Ensures patients have scheduled follow-up appointments prior to discharge.
  • Participates in performance improvement activities, analyzing and interpreting data.
  • Assists with prioritizing and assigning patients to correct providers.
  • May coordinate care outside of the hospital for patients not requiring hospitalization, focusing on prevention of readmissions and preventable admissions.
  • Serves as a resource to clinical staff providing direct patient care services with related issues.
  • Answers pharmacy, patient, family, and care provider calls post-discharge to assist with transitions.
  • Provides follow-up on pending labs, procedures, or tests post-discharge.
Physical Requirements

Walking and heavy lifting required; must support ten pounds shoulder high; must speak English fluently; good sight and hearing; critical thinking skills; ability to respond quickly in a fast-paced environment.

Education, Experience, and Certifications

Graduate from an accredited School of Nursing; BSN preferred; 3 years nursing experience; ICU or ED experience preferred; current BLS for Healthcare Provider; RN license or temporary license required in the state of work; additional certifications or training may be required by nurse leader.

Pay Range

$34.90 - $52.35
