RNLPN

We are seeking an RN/LPN to join our team! You will be responsible for the assessment, diagnosis, and treatment as ordered of assigned patients.

Responsibilities:

• Provide nursing care in accordance with the client's plan of care, including comprehensive health and psychosocial evaluation, monitoring the client's condition, health promotion and prevention, coordination of services, teaching, training activities, and direct nursing care.
• Coordinate total client care by conducting comprehensive health and psychosocial evaluations, monitoring the client’s condition, promoting health and preventive practices, coordinating services, and conducting teaching and training activities.
• Monitor and report changes in patient symptoms or behavior.
• Communicate with collaborating physicians or specialists regarding patient care.
• Educate patients and families about health maintenance and disease prevention.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Maintain accurate patient medical records, submit clinical and progress notes timely, and present for clinical record review.
• Provide advice and emotional support to patients and their families.
• Participate in client’s case conferences and discharge planning processes.

Qualifications:

• Must be a graduate from an accredited school of nursing and licensed as a registered or practical nurse in North Carolina.
• Minimum of one year of experience.
• Previous experience in nursing or other medical fields.
• Familiarity with medical software and equipment.
• Ability to build rapport with patients.
• Strong problem-solving and critical thinking skills.
• Ability to thrive in a fast-paced environment.
