Job Title: RN Wellness Nurse

Supervisor: Clinical Services & ADHC Program Director

FLSA Status: Non-Exempt

Job Summary

The Wellness Nurse has the responsibility of providing direct and indirect nursing care to assigned patients. Works under the direct supervision of the Clinical Services & ADHC Program Director but is independent in the application of basic nursing knowledge and skills.

Specifications

Education: Graduate of an accredited school of nursing.

Experience: Has practiced nursing in the last 3 years. Has 3 years of RN/LPN experience with at least 1 year of experience working with the frail, elderly population.

Number and Type of Employees Supervised (optional): < 20 ADHC Employees.

Licensure, Registry or Certification

Required:

RN/LPN; licensed in NC.

Special Training: Must possess knowledge and skills necessary to treat patients; other training as required by department. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior TLC and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Age of Patients Rendered Care:

Key Responsibilities
  • Observation & Documentation:
    Collect patient data and complete required forms with appropriate responses according to unit standards.
  • Communicate with customer service liaison regarding participant health concerns as needed using the triage tool.
  • Identify patient’s overt problems/needs and set priorities.
  • Identify problems requiring further referral and/or follow-up.
  • Perform advanced nursing observation using critical thinking skills.
  • Identify and monitor acute participant changes. Communicate participant changes to providers in a timely and effective manner.
  • Observe any participants to ascertain if the participant needs to be seen in the clinic.
  • Perform general nursing care on participants, including treatments as needed.
  • Administer medications, provide training in self-administration of all non-scheduled medications and monitor medication compliance.
STLC Day Center Delivery of Care
  • Works in the STLC Day Center Activity rooms, providing routine daily care including but not limited to medications, blood sugar monitoring, etc.
  • Communications with participants, providers, and RN Care Coordinators to ensure team and care givers are notified.
  • Monitors participant attendance and follow up on unexplained absences, and forwards the information to the appropriate care team.
  • Performs rounds at the STLC Day Center for the purpose of triaging and administering to the concerns of participants and Day Center staff.
  • Documents, charts, and communicates results from rounds thoroughly to appropriate IDT member(s).
  • Care provided conforms to accepted practice standards and patient care standards.
  • Demonstrates understanding of age-related characteristics and needs of patients served.
  • Explains nursing procedures in an understandable manner.
  • Communicates with family/caretaker regarding STLC Day Center delivery of care.
  • Provides participant and family education in coordination of 24-hour care delivery.
  • Identifies emergency situations and independently initiates advanced therapy.
  • Implements and monitors Plan of Care, identifying and communicating changes in condition to appropriate IDT member(s).
  • Understands and demonstrates respect for participant rights and utilizes established mechanisms for management of ethical issues in participant care.
  • Implements nursing measures related to impending or associated problems.
  • Monitors monthly health/wellness education provided to participants at STLC Day Health & Wellness Center.
  • Diabetic foot measuring and ordering diabetic shoes.
  • Preventative diabetic foot assessments.
  • Acute nurse visits for participants attending ADHC.
Supervisory Responsibilities
  • Directs the staff to meet clinical assessment guidelines.
Leadership And Communication
  • Provides clinical teaching and clinical supervision of STLC Day Center Staff.
