We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes in the Charlotte, NC area. This role focuses on performing comprehensive patient assessments, coordinating care in accordance with physician orders, and managing the full home health patient lifecycle, including admissions, re-certifications, and discharges.
The ideal candidate is comfortable working independently in the field, has strong clinical judgment, and is experienced in documentation and care coordination within a home health or hospice environment.

Key Responsibilities
• Perform initial home health visits, ongoing assessments, re-certifications, and discharge evaluations
• Admit, transfer, re-certify, and discharge home health patients in accordance with physician orders
• Develop, implement, and update individualized plans of care
• Monitor patient progress, observe changes in condition, and document findings accurately
• Educate patients and caregivers on disease processes, medications, and home care techniques
• Coordinate care with physicians, case managers, therapists, and other clinical staff
• Supervise and provide oversight to LPNs and Home Health Aides (HHAs)
• Maintain accurate and timely clinical documentation in compliance with agency policies and state regulations
• Participate in case conferences to ensure continuity and quality of care
• Discharge patients from skilled nursing services when goals of care are met
