Essential Functions
- Delivery of Patient Care
- Assume responsibility for coordination and delivery of patient/caregiver care.
- Conduct nursing assessments to assist in overall management of patient/caregiver needs.
- Develop, implement, evaluate, and update the plan of care as appropriate.
- Facilitate patient and caregiver independence to the extent possible.
- Use VIA Health Partners protocols, standing orders, and guidelines when providing hospice care.
- Conduct effective patient and caregiver education, encouraging questions and consultation.
- Provide education to patients and families as needed.
- Communicate effectively with IDG members, patients, caregivers, and other health professionals to ensure continuity of care.
- Maintain open lines of communication with supervisor, keeping them apprised of complex issues.
- Apply infection control principles and follow universal precautions.
- Interdisciplinary Group
- Attend, participate, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.
- Establish and maintain effective working relationships with team members.
- Identify specific patient/caregiver needs and refer to appropriate team members.
- Documentation
- Ensure accurate, complete, succinct, and timely clinical documentation per VIA Health Partners guidelines and licensure requirements.
- Professional and Personal Accountability
- Demonstrate competency in knowledge and skills.
- Maintain VIA Health Partners professional reputation.
- Engage in professional development and stay abreast of current trends.
- Maintain current professional licensure, CPR certification, and CEU requirements.
- Maintain productive and professional relations with external agencies and healthcare providers.
- Exhibit time‑management skills and respond to crises appropriately.
- May perform other duties as required
- Clean patient rooms per patient‐room cleaning procedure and keep facility areas clean and stocked.
- Maintain cleanliness of the family room throughout the day.
- Use proper chemicals and protective equipment when cleaning surfaces.
Qualifications
- A diploma or associate degree in nursing is required; a bachelor’s degree preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- Current CPR required; CHPN preferred.
- Minimum 1‑year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life‑limiting illnesses required; may include hospice, palliative care, family medicine, internal medicine, geriatrics, oncology, home health, or medical‑surgical settings.
- Computer proficiency is required.
