PrEP Case Manager - Public Health

Hiring Range: $52,132.00- $68,423.25

This is an exempt (salaried) position. Pay rates are based on education, skill, experience level and internal equity. Internal equity consideration included an assessment of the applicant’s salary history and qualifications in comparison to the market rate and requirement for the job.

This role is on-site 5 days a week, 8:00 AM to 5:00 PM.

This is a Grant Funded Position through May 31, 2026, with renewal options through May 31, 2029 based on available funding.

Position Summary
This position is assigned to the Mecklenburg County Public Health (MCPH) Pre-Exposure Prophylaxis (PrEP) Clinic. The primary responsibility is to provide supportive services to patients enrolled in thePrEP Program. The PrEP Case Manager is an important member of the patient services team and works closely with the clinical team to provide excellent patient centered care. The Case Manager works with patients from the point of referral and provides on-going Case Management to ensure retention in services.

The Case Manager will also assist with case finding and support outreach and program promotion.

Essential Functions
Provide PrEP education and HIV/STI Risk Reduction Counseling to person interested in receiving PrEP.
Assist new and continuing patients with initial and follow-up medical appointments.
Identifies patients at risk for falling out of PrEP care and those with unaddressed quality-of- care items which elevate the risk of disengagement.
Assist with referrals and coordinates external services as needed to mitigate risk of disengagement.
Facilitates completion of prior authorization documents for PrEP medication access assistance.
Accurately documents all patient encounters within appropriate EMR and/other designated systems.
Collaborates with clinical team members to coordinate patient services.
Maintains and optimizes all forms of patient communication (portal, phone, etc.) to engage with patients on a regularly scheduled basis to ensure continuity of access and retention.
Prepares reports and performs administrative duties necessary for grant reporting.
Participates in emergency preparedness activities as needed.
Completes survey data reports as needed.

Minimum Qualifications
Experience:

Minimum of two years of related public health experience.

Education:
Bachelor’s degree in public health or related field.
Combination of relevant education and relevant experience accepted? Yes
Licenses and Certifications
Requires a valid North Carolina or South Carolina Driver’s License
Requires County Driving Privileges

Preferred Qualifications
Currently Certified as a HIV PrEP Navigator (Health HIV) or willing to complete HIV PrEP Navigator certification upon hire

Knowledge, Skills and Abilities
Knowledge of

Principles and practices of public health
Local resources and services
The disease concepts of HIV infection, AIDS, Syphilis and other STDs
Person-centered planning

Skills
Client interviewing, disease management, and client centered counseling
Excellent oral and written communication skills
Basic computer programs, word processing, and data entry, and job specific software
Establish rapport and effective communication successfully among culturally diverse populations
