PURPOSE:
The LCSW serves as The
C. W. Williams Community Health Center’s (CWWCHC) Behavioral Health (BH) expert. The LCSW provides a variety of BH services within their scope of practice. In addition, may serve on various committees within and outside of CWWCHC related to integrated health care issues. The LCSW will model behaviors consistent with the published values and the Code of Conduct of CWWCHC.

- Assesses patients for behavioral health needs utilizing screenings and assessments that are nationally acclaimed and standards of care.
- Diagnoses patients as allowed by the scope of practice.
- Provides evaluation of all patients and documents said evaluation into the Electronic Health Record (EHR).
- Provides crisis intervention for the organization.
- Provides brief intervention/treatment as needed.
- Provides consultations with patients both at Point of Care as well as a routine appointment.
- Identifies additional behavioral health needs and refers patient accordingly.
- Provides care coordination/care management and education to patients and staff members as required.
- Provides advocacy and other related outpatient services.
- Licensed as a LCSW by the state of North Carolina
- Three years of behavioral healthcare required
- Experience in Primary Healthcare Setting preferred
- Must possess tact, discretion, and diplomacy
- Critical thinking, problem- solving, and conflict resolution skills are essential
- Must possess a working knowledge of the current Diagnostic and Statistical manual of Mental Disorders (DSM)
- Must be able to use protocols, guidelines and benchmarks to implement policies and develop quality improvements while utilizing best practices
- Excellent organizational skills, leadership skills, and excellent verbal and written communication skills
- Proficiency using EHRs systems and other computer programs
- Must be able to effectively handle multiple tasks and priorities in order to meet deadlines
40 hours per week and as requested by Supervisor.
SalaryAs specified in the Notice of Hire.
Perform Other Duties as Assigned.
