We are seeking a dedicated and compassionate individual to join our team as a Licensed Qualified Professional for Intensive In-Home Services. This position is integral to the delivery of care within our Intensive In-Home Team, designed to make a significant impact on the lives of the families we serve. The role requires a commitment to improving the quality of life and fostering stronger connections within the community for our clients.
Whatyou will do
As a Qualified Professional for Intensive In-Home Services, under the supervision of the Team Lead, you will be responsible for providing therapeutic interventions to children and adolescents with severe emotional and behavioral challenges. You will conduct comprehensive assessments, develop person-centered treatment plans, and work collaboratively with a multidisciplinary team to ensure that our clients receive holistic, effective care.
Your responsibilities will include:- Conducting initial and ongoing assessments to identify strengths, needs, and goals of the clients and their families.
- Developing, implementing, and monitoring individualized treatment plans in coordination with the family, school, and community resources.
- Providing intensive in-home interventions and support to clients and families to help them manage and overcome their challenges.
- Supporting clients in crisis situations, developing safety plans, and coordinating with emergency services when necessary.
- Educating families on behavioral management strategies and connecting them to community resources.
- Advocating for clients within the educational, judicial, and social service systems.
- Maintaining accurate and timely documentation of all services provided.
- Collaborating with other team members to ensure continuity of care and to promote positive outcomes for clients.
- Assisting with person-centered plans, NC Topps submissions, and other items directed from Team Lead.
Additionally, you will have the opportunity to exercise autonomy within your role while actively participating in the growth and development of our service area through community engagement and partnerships with various behavioral health resources.
Qualifications to join a winning teamIf you are passionate about making a difference in the lives of families and have the following qualifications, we encourage you to apply:
- A Master degree in human service and hold a provisional license in LCMHC or LCSW.
- At least 1 year experience working with children and adolescents with severe emotional and behavioral issues.
- Strong skills in assessment, crisis intervention, and family systems therapy.
- A valid driver’s license and reliable transportation.
