Job Title: Care Navigator
Department: Clinical Outpatient
Reports To: Clinical Supervisor
FLSA Status: Hourly/Non-Exempt
Anuvia is seeking a dedicated Care Navigator to join our Clinical Outpatient team. This role is central to ensuring our clients receive safe, effective, and client-centered care. The Care Navigator monitors client needs, coordinates treatment plans, connects clients with community resources, facilitates care transitions, and advocates for client well-being. This position also supports admissions, service authorizations, and documentation requirements.
Key Responsibilities- Coordinate and deliver care that is timely, effective, equitable, and client-centered.
- Conduct client admissions, obtain service authorizations, and develop person-centered treatment plans.
- Review assessments and referrals to ensure appropriate service connection.
- Monitor treatment progress, signs of substance use withdrawal, and adjust care as needed.
- Facilitate discharge and transition planning.
- Provide education on substance use, health, and community resources.
- Assist clients with applications and service-related documentation.
- Build and maintain strong relationships with referral agencies and community partners.
- Maintain accurate, timely documentation in accordance with state, agency, and federal guidelines.
- Administer medications per provider orders (training provided) and take vital signs as directed.
- Adhere to all ethical standards, codes of conduct, and agency protocols.
- Participate in staff meetings and multidisciplinary team discussions.
- Bachelor’s degree preferred, or equivalent experience/training.
- Must meet criteria for Qualified Professional and maintain certification with the North Carolina Addictions Specialist Professional Practice Board and/or North Carolina Department of Health and Human Services
. - Valid NC or SC Driver’s License required.
- Strong interpersonal, problem-solving, and communication skills.
- Ability to document in Electronic Health Records and use Microsoft Office applications.

- Based in our Clinical Outpatient department with community-based work as needed.
