Overview
Summary:
Under the supervision of the Program Manager and/or Team Leader, provides case management, rehabilitation, and crisis interventions and outreach services in home, office, and community settings to adults with serious long-term mental illness. Collaborates with psychiatric and allied mental health team in assessment, diagnosis, intervention and treatment planning by performing the following duties. Conduct substance abuse treatment with clients in need in accordance with the 12 core functions of substance abuse counseling as defined by the ICRC.

- Provides clinical interventions such as resource linking, resource modification and development, skill teaching and programming to consumers on Mobile Treatment Team caseload.
- Assists consumers in obtaining financial benefits, medical and dental care, employment, decent housing, social support networks, social and leisure activities, and ADL skills in real-life environments.
- Conducts comprehensive clinical assessments and develops interventions for substance abuse, sexual abuse, symptom management, and family support and education.
- Assesses suicidal and homicidal behavior.
- Coordinates emergency psychiatric services in a timely manner, including documenting interventions, alternatives considered, and responses of clients, families, and supporting agencies.
- Provides, coordinates, and monitors treatment services for all assigned cases, including:
- 22 hours of direct services (face to face) with clients to meet productivity requirements;
- Documentation in progress notes following established procedures as described in the Individual Treatment Plan;
- Provision and coordination of care prescribed in the Treatment Plan through documented contact and implementation with the client/family, Gateway staff, consultants, and staff from schools and other organizations;
- Participates in treatment planning by completing assessment assignments within time requirements, scheduling treatment planning meetings, and utilizing assessment information and results to develop treatment plans.
- Provides psycho-educational services to clients and families based on plan specifications, including information on mental illness, substance abuse, effective parenting, and behavior management.
- Arrives on time for therapeutic services; follows prescriptive instructions from the Individual Treatment Plan and adapts plans as client needs change.
- Documents treatment implementation with the client in progress notes and/or treatment plan reviews; assists with discharge planning.
- Ensures medical record documentation aligns with Gateway policies, MTT Medicaid guidelines, client confidentiality, and client rights procedures.
- Attends scheduled meetings on time and actively participates in agency and inter-agency meetings as designated; contributes to daily staff organizational and treatment planning review meetings.
- Provides ongoing assessment of
client mental illness symptoms and response to treatment; delivers problem solving, side-by-side services, skill training, supervision, and environmental adaptations to support activities of daily living. - Assists clients in finding and maintaining safe, affordable housing, budgeting, transportation, and access to primary care and dental services.
- Plans and supports social and leisure activities, and prepares for supervision by completing assigned tasks and formulating questions.
- Monitors medication to ensure understanding of prescribed dosage, side effects, and adherence.
- Attend weekly general staff meetings and perform other duties as assigned.
- Associate s degree required;
Bachelor s degree preferred. - Two years of experience in Human Services preferred.
- Experience working with individuals with substance abuse, sexual abuse, and/or involvement with the criminal justice system preferred.
Special Requirements
- Valid driver s license and use of car during normal working hours or other means of transportation to perform major job functions.
- Automobile liability insurance coverage for vehicle used in accordance with state regulations.
Pay Range: $19.58-$32.31
