Roleand Location
Psychiatric Nurse Practitioner (PMHNP) to join our team on a full-time basis
. We are seeking a dedicated and compassionate PMHNP to help provide exceptional care and support to our geriatric patients in the LTC community.
Location:
Charleston, WV
.
Compensation: $110,000 - $130,000 per year plus bonus opportunities
Hours: Full-time
Responsibilities- Conduct Comprehensive Psychiatric Evaluations: perform thorough psychiatric assessments, manage psychotropic medications, and deliver personalized, need-based care.
- Ongoing Medication Management: develop and adjust treatment plans based on patient progress and input from the interdisciplinary team.
- Collaborative Care Planning: partner with case managers and interdisciplinary teams to create and refine care plans for integrated behavioral health management.
- Team Engagement: participate in regular team meetings to evaluate new referrals, review active cases, and adjust care plans.
- Engagement in Quality Programs: contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
- Travel: travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide support across clinical settings.
- Master’s or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health
- Current and unrestricted PMHNP license in the state of WV
- Board certification as a PMHNP-BC
- Active and unrestricted DEA license (We will help obtain)
- Clinical Expertise: proficiency in psychiatric evaluations, psychotropic medication management, and evidence-based treatment planning.
- Interpersonal and Team Collaboration
Skills:
ability to work with interdisciplinary teams in diverse care settings. - Community-Focused: awareness of social determinants of health and access to resources to address holistic patient needs.
- Tech-Savvy: comfortable with EHRs, telehealth platforms, and other digital tools.
