We are seeking an Intake and Assessment Clinician – Masters for our team to ensure that we continue to provide all patients with high quality, efficient care. We want you to apply!

Job Summary & Qualifications
  • Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care.
  • Identifies cultural considerations, gender identity and preferred language for patients and family.
  • Demonstrates advanced practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope).
  • Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient’s condition.
  • Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
  • Recognizes and acts upon unexpected assessment findings.
  • Leads in ensuring a safe structured milieu with evidence‑based treatment modalities/resources.
  • Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers.
  • Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.
  • Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team.
  • Develops and maintains a community resource guide including access to public assistance/alternatives.
  • Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope).
  • Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
