RN Case Manager; PRN

Overview

The Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.

The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management.

• Coordinate a plan of care to achieve clinical outcomes and promote safe, timely discharge.
• Manage the plan of care across the care continuum to support efficient patient movement through the hospital system.
• Serve as a clinical expert in discharge planning, resource utilization, and case management under the guidance of a Team Leader.

Bachelor’s degree in Nursing from an accredited school of nursing and three years of work experience as a Registered Nurse required. Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred.

Licensure as a registered nurse by the state of South Carolina or a compact state required. Current Basic Life Support (BLS) or ACLS required, either through an American Heart Association (AHA) BLS for Healthcare Providers or an AHA-recognized equivalent, or American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.

Ability to perform job functions while standing, sitting, and walking (continuous). Ability to climb stairs (infrequent). Ability to work indoors and outdoors in temperature extremes (infrequent). Ability to work from elevated areas (frequent) and in confined/cramped spaces (frequent). Ability to perform repetitive motions with hands/wrists/elbows and shoulders (continuous). Ability to lift and carry up to 50 lbs (infrequent) and to lift/lower objects from floor to 36 inches (infrequent).

Visual and auditory requirements: maintain 20/40 vision, corrected; good peripheral vision; maintain hearing acuity with correction. Ability to work rotating shifts and overtime as required. Ability to work in a latex-safe environment.
