- Patient Care Coordinator The patient care coordinator is a key role at Deeper Healing. The position is full-time, and in-office at our Daniel Island Center. The patient care coordinator handles the questions and concerns of prospective patients, walks new and existing patients through the specifics of their healing program and provides follow up after physician or nurse practitioner visits (in-office and remote) including recommended treatments, scheduling and timing, costs, financial options and next steps.
Coordinators assist our remote patients to strategize travel arrangements and work closely with them to make sure they feel well supported throughout the process.
Success:
Making sure patients are well informed, excited to start, booked for appointments and packages procured. Need to be detail oriented, comfortable with educating all ages and stages and discussing costs and timing.
The position is both educational and consultative. The patient care coordinator will possess excellent people skills, be articulate, compassionate, and above all comfortable with the challenges that accompany chronically ill people. They possess excellent customer service, interpersonal and relationship building skills, display a positive attitude while conducting oneself with professionalism and integrity in all aspects of the job, have strong verbal and written communication skills, ability to organize, plan, multi-task and remain calm in a rapidly changing environment, have computer proficiency and work well in a team.
Responsibilities:- Demonstrate and influence a culture of good teamwork to deliver an extraordinary experience for every patient
- Get Vitals on new patients
- Keep track of where patients are in the process and stay in touch, phone, email, emr
- Address patient’s questions and/or concerns, and know when to collaborate with practitioners/management
- Respond to patient requests as quickly as possible
Maintain patient confidentiality through HIPAA compliance - Follow all safety policies and procedures
- Use the EMR and procedures to keep patients up-to-date
