Outpatient Intensivist opportunities available- full and part-time - Greer Transitions Clinic
Seeking fulltime and part-time intensivists to provide primary care for critically/chronically ill patients after ED discharge through the Roper St. Francis Physician Partners Transitions Clinic locations.

- Fulltime:
Four days in clinic, one Admin day weekly - Part-time:
Two days in clinic weekly - North Charleston and West Ashley locations

- Patients are insured, underinsured and uninsured. Resources and charity funding available for patients through the Roper St. Francis Healthcare Foundation
The Greer Transitions Clinic improves care coordination as a one-stop-shop where underfunded patients with no primary medical home can visit with board-certified physicians, receive preventative care, connect with social services, receive financial assistance, transportation subsidies, birthing classes, food, clothing and so much more.
The clinic has been transformational to the residents of North Charleston, seeing nearly 2,000 patients a year and reducing emergency department visits for patients of the clinic.
