Position Summary

Our MDS Coordinator is responsible for coordinating MDS as required per Federal guidelines. The role includes providing residents with direct care to assure quality of life in a home‑like environment while offering additional support to family and visitors. The Coordinator maintains professional behavior while functioning as an interdisciplinary team member and meeting state and federal guidelines.

Essential Functions
  • Ensure all necessary written documents—Admission Nursing Assessments, Nursing Notes, Over‑All‑Care Plans, and other required information—are kept current in the resident record system and comply with facility policies, ASI standards, and state/federal regulations.
  • Assist the Directorుణ of Nursing in maintaining facility standards to meet state, federal, and ASI requirements and assist with inspections.
  • Determine that medication systems meet resident needs, are effective, and comply with state regulations and pharmaceutical guidelines.
  • Inform the Administrator of significant changes in resident conditions, hospital transfers, expirations, or any pertinent information concerning интеллекту личений.
  • Assist in determining which residents are admitted using established nursing care criteria; make placement decisions and transfer residents as necessary.
  • Communicate with the Administrator to schedule and conduct nursing staff meetings and maintain maintenance records.
  • Assess supply and equipment needs for the nursing department and ensure adequate stock.
  • Determine that incident reports are completed as necessary and review them routinely.
  • Remain available for call when needed by staff cyclone duty.
  • Perform any other duties as may be necessary, as specified by the facility Administrator.
  • Complete or Patricia the following assessments: skin, contracture, fall, incontinence, catheter, side‑rail, etc.; complete assigned MDS sections; produce RAP summaries for urinary incontinence, falls,…
