Position: Developmental Disability Spec I- Support Coordination

This position works within our Support Coordination Division. You would be part of a dynamic multidisciplinary team delivering support coordination services to individuals with intellectual or other developmental disabilities. Using a collaborative person-centered planning approach, assist the individual in accessing needed medical, psychiatric, social, educational, vocational, residential and other supports essential for living in the community and in developing his/her desired lifestyle.

Complete assessments, lead the interdisciplinary team in developing the person-centered plan with the individual, link to needed services/supports, and monitor delivery and quality of services according to the plan and changes plan as needed. Act as a resource to the individual and his/her family and provide information about county, generic, and community resources. Meet with individuals in the community where services are provided.

Collaborate with vendors to ensure health and safety needs are met. Manage emergencies. Requires use of PC and electronic health record; requires excellent written and verbal communication skills.

Key Responsibilities
  • Coordinate and lead interdisciplinary team meetings to develop and monitor consumer service plan.
  • liDetermine most appropriate services and coordinate with service provider and family to schedule multidisciplinary evaluation.
  • Participate in implementation and review of individual service plans.
  • Participate in meetings related to services and needs.
  • Provide crisis intervention in emergency situations and collaborate with supervisor regarding need for intervention by external sources as necessary.
  • Link clients with appropriate community resources.
  • Organize and maintain individual files and other confidential records, including state plan option and non‑Medicaid case records.
  • Provide continuous review and evaluation of individual's progress towards established goals.
  • Represent program in the community.
  • Maintain ongoing communication with families, county programs and agencies, and state agencies.
Key Knowledge,

Skills and Abilities
  • Knowledge of principles, practices, methods, and techniques related to treatment of individuals with an intellectual disability or other developmental disabilities.
  • Knowledge of psychiatric, psychological, and/or sociological terminology and concepts.
  • Knowledge of counseling and interviewing techniques.
  • Knowledge of case management principles.
  • Knowledge of federal, state, and local laws and regulations, as well as issues relevant to human rights, mandated reporting, and skill in applying this knowledge to a variety of situations.
  • Knowledge of program services and community resources, including relevant public and private organizations.
  • Ability to successfully participate and/or facilitate interdisciplinary team meetings.
