Field Nurse Practitioner to provide compassionate, high‑quality care to patients across community‑based settings. This is not a remote position—you’ll deliver in‑person care throughout Maricopa County, traveling to patient locations as needed.

• Provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings in Maricopa County.
• Order, interpret, and evaluate diagnostic tests to assess and manage patients’ health conditions.
• Deliver health education, counseling, and disease prevention guidance to promote wellness.
• Collaborate with physicians, nurse practitioners, and other healthcare professionals as part of an interdisciplinary care team.
• Develop, document, and update individualized care plans.
• Prescribe medications and other appropriate treatments in accordance with licensure and scope of practice.

• MS in Nursing.
• Minimum of 1 year of clinical experience as a nurse practitioner, or equivalent education‑experience combination.
• Current unrestricted RN license and NP license in applicable state.
• Completion of a Tuberculosis test.
• Position is bonus eligible based on individual and company performance.
