DDD Care Manager

We are seeking a Support Coordinatro to join our independently owned and operated national agency. This individual must be eager to make a difference and be willing to be cross trained in other areas of the business.

• Competitive pay ranges between $50,000-$62,000.

• Schedule caregivers with clients and communicate to all parties
• Conduct/train Article 9 and prevention and support classes to all employees
• Conducts daily/weekly reporting
• Conducts quality calls and quality visits
• Confirm and update caregiver availability including vacation, sick time, and holidays
• Varifies payroll daily
• Establishes and build realtionships with contract providers to gain new clients and understand their needs
• Respond to employment leads regarding open positons with possible clients
• Document skills and attributes of caregivers to ensure strong matching to clients
• Follow up with new caregiver hires to explain the scheduling process
• Establish a great rapport with caregivers and clients
• Obtain ISP updates and enter HAB goals as per the ISP
• Ensure that authorization is in place prior to services starting
• Is on call as per on call calendar requirements
• Follow up on all new leads, track needs and staff appropriately

• Experience – One year experience in a home care setting preferred, and understand the DDD industry, and be willing to obtain Article 9 instructor certifications
• Education – Associates degree in any field
• Skills – Exceptional customer service skills, time management skills, excellent communication skills, willingness and able to become if not already an Article 9 and prevention and support instructor. Must be able and willing to train, mentor, interview and hire all new team members.
