PositionRN Team Supervisor — 2 positions available (1 with a Behavioral Health focus)
Responsibilities- Provide independent RN visits as appropriate
- Perform clinical triage (in-person, phone, portal)
- Manage chronic conditions under standing orders and protocols
- Review and act on labs, diagnostics, and clinical data per protocol, delegating when appropriate
- Prepare for clinic session by scrubbing the schedule and pre-visit charting
- Lead/participate in a daily huddle with the team to review day-of-care plan for scheduled patients
- Provide direct patient care based on patient need (e.g., urgent vs routine follow-up vs annual health assessment)
- Clearly communicate patient care priorities, including follow-up plans involving other PCMH team members
- Initiate specialty care referrals per protocol
- Document care plan in the EHR in a timely fashion
- Review no-shows/cancellations at the end of sessions with plans for follow-up and communicate with the team to contact patients
- Serve as care manager for assigned patient panels
- Coordinate transitions of care (ED, hospital, specialty care)
- Collaborate with providers, behavioral health, pharmacy, and community resources
- Close preventive and chronic care gaps using population health tools
- Demonstrate good interpersonal skills and treat all patients and colleagues with respect and dignity
- Act as clinical supervisor for assigned LPN/MA clinical assistants and MAs on the team
- Delegate clinical and non-clinical tasks based on scope and competency
- Provide real-time coaching, feedback, and competency validation
- Conduct daily huddles with assigned teams
- Ensure delegated tasks are completed safely and appropriately
- Collaborate with other RN Clinical Supervisors for coverage needs
- Monitor quality metrics for assigned panels in conjunction with the Director of Quality
- Participate in quality improvement and Patient Centered Medical Home (PCMH) activities
- Engage in quality improvement initiatives
- Ensure compliance with nursing standards, protocols, and policies
- Promote patient safety and evidence-based practice
- Assist in designing and implementing clinical pathways and protocols for treatment of selected chronic conditions
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Proficiency with electronic health record systems
- Ability to work independently with minimal supervision
- Highly organized with strong oral and written communication skills
- Demonstrated critical thinking, planning, and execution abilities
- Effective leadership marked by integrity, fairness, and accountability
- Ability to work with individuals from diverse cultural and socioeconomic backgrounds
- Excellent communication, interpersonal, and customer service skills
- Ability to manage multiple priorities in a fast-paced environment while maintaining confidentiality
- Graduation from an accredited RN program
- Two plus years of directly related clinical experience in a medical setting as an active team member is highly preferred
- Maintains required licensure or certification as appropriate
- Comfortable with the pace of primary care and working with an interdisciplinary team
- Extensive working knowledge of Next Gen EMR system highly preferred
- CPR certified (BLS) or ability to obtain
- Knowledge of Microsoft programs with the ability to effectively communicate using Outlook/TEAMS
