RN Team Supervisor - 2 positions available (1 with a Behavioral Health focus)The RN Team Supervisor functions as the clinical leader of an assigned care team and is accountable for nursing care delivery, delegation, and outcomes for an assigned patient population. The RN practices to the full extent of licensure, provides clinical supervision to assigned clinical assistants (LPN/Medical Assistant), Medical Assistants and leads team-based care focused on quality, access, and population health outcomes.
This role is not task-based; it is assessment-, judgment-, and leadership-driven.
ESSENTIAL DUTIES & RESPONSIBILITIES Clinical Care- Provide independent RN visits as appropriate
- Perform clinical triage (in-person, phone, portal)
- Manage chronic conditions under standing orders and protocols
- Review and act on labs, diagnostics, and clinical data per protocol, delegating when appropriate/warranted
- Prepares for clinic session by scrubbing the schedule and pre-visit charting
- Leads/participates in a daily huddle with team to review day-of-care plan for scheduled patients
- Provides direct patient care based on patient's need (e.g. urgent vs routine follow up vs annual health assessment)
- Clearly communicates patient care priorities, including follow-up plans that involve other PCMH team members
- Initiates specialty care referrals per protocol
- Clearly documents plan in EHR in a timely fashion
- Reviews no shows/cancellations at the end of sessions with plans for follow up as appropriate and communicates with team to contact patients.
- Serve as care manager for assigned patient panel(s)
- Coordinate transitions of care (ED, hospital, specialty care)
- Collaborate with providers, behavioral health, pharmacy, and community resources
- Close preventive and chronic care gaps using population health tools
- Good interpersonal and human relations skills are required. Must possess skills to empathize and care for a variety of patients, including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity.
- Acts as clinical supervisor for assigned LPN/MA clinical assistant, and MAs assigned to his/her team.
- Delegates clinical and non-clinical tasks based on scope and competency
- Provides real-time coaching, feedback, and competency validation
- Conducts daily huddles with assigned teams
- Ensures delegated tasks are completed safely and appropriately
- Works with other RN Clinical Supervisors for coverage needs
- In conjunction with the Director of Quality, monitors quality metrics for assigned panel(s)
- Participates in quality improvement and Patient Centered Medical Home (PCMH) activities
- Participates in quality improvement initiatives
- Ensures compliance with nursing standards, protocols, and policies
- Promotes patient safety and evidence-based practice
- Has the ability to help design and implement clinical pathways and protocols for treatment of selected chronic conditions
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Proficiency with electronic health record systems.
- Ability to work independently with minimal supervision of VP of Operations/Assistant Director of Operations.
- Highly organized and well-developed oral and written communication skills.

- Ability to manage multiple priorities in a fast-paced environment while ensuring confidentiality
- Graduation from an accredited Registered Nursing (RN) program.
- Two plus years of directly related clinical experience in a medical setting as an active team member is highly preferred.
- Maintains required licensure or certification as appropriate.
- Is comfortable with the pace of primary care, working with an interdisciplinary team, and has strong communication skills.
- Extensive working knowledge of the Next Gen EMR system highly preferred.
- CPR certified (BLS) or ability to obtain.
- Knowledge of Microsoft programs with the ability to effectively communicate using Outlook/TEAMS.
