We are seeking a Social Services Director to lead our department and serve as a key member of the interdisciplinary team. This role ensures residents’ mental, emotional, and psychosocial needs are met and supports safe, effective discharge planning.
Base pay range$22.00/hr - $22.00/hr

- Bachelor’s degree in Social Work
- 2–3 years’ experience in social services or discharge planning in a long-term care/skilled nursing setting
- Knowledge of Medicare/Medicaid & insurance
- Complete assessments and develop individualized care plans
- Participate in interdisciplinary care plan meetings
- Document admissions, discharges, and transfers
- Support healthcare directives and end-of-life planning
- Lead discharge planning with residents, families, and care teams
