Person in this position functions on a multidisciplinary team to provide care to persons with serious and persistent mental illness in community based settings.

• Licensed Illinois RN.
• Experience in psychiatric nursing or with mental health populations preferred.
• Knowledge of psychotropic medications.
• Excellent written and oral communication skills.
• Ability to quickly establish rapport.
• Computer proficiency required, including navigating and completing documentation in the electronic health record.
• Ability to accurately complete required documentation within the prescribed time frames.
• Ability to present and coordinate lectures for clients, parents, staff or community resources.
• CPR training or complete within three months.
• Valid driver’s license and ability to meet agency insurance requirements (designated programs).
• Must have own transportation (designated programs).
• Belief in the mission and vision of Rosecrance.

• Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
• Demonstrate essential nursing skills; respond to medical crises when on site.
• Assess physical/medical needs of assigned program clients.
• Coordinate medical appointments for assigned program clients; provide transportation and support when needed.
• Complete medication administration and training for program clients.
• Complete prior authorizations as needed for clients’ medications.
• Maintain communication with team leader and coordinator on medication assistance requests and budgets.
• Build relationships with medical community and providers; advocate for program clients to obtain appropriate care.
• Provide education to program clients regarding medical problems and prevention issues.
• Provide education on nutrition, wellness, medications and symptom management to program clients either group or individual sessions.
• Complete required Rule 132 documents including progress notes, MHA’s, tx plans and reviews.
• Be familiar with court proceedings and available to make court appearances on behalf of Rosecrance clients.
• Serve as a member of the team and participate in all team meetings and activities.
• Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
• Deliver exceptional customer service consistently to every customer.
• Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
• Assume other related responsibilities as delegated and assigned by management.

• Hours:
  
  Monday - Thursday 8:30am - 7:00pm (40 hours per week).
• Shift: 1st shift.

Rosecrance Walnut, Champaign, IL. Onsite (Community Based).

• Starting pay based on education, experience, and credentials.
• RN - $32.65 per hour.
• BSN - $34.02 per hour.
