Reliance Hospice and Palliative Care is seeking a detail-oriented LVN or RN Hospice Admissions Manager with an outgoing personality and a passion for teamwork to oversee and coordinate our patient admissions process. This role will support the admissions team and ensure excellent patient care and smooth intake procedures.
Hospice experience is required.

Hourly salary starts at $38 for LVN and $53 for RN andis negotiable depending on years of Hospice experience.

Key Responsibilities:

• Oversee and coordinate all aspects of patient referral and intake processes.
• Supervise admissions staff and assist with scheduling and staffing functions.
• Maintain accurate and up-to-date patient charts and records in the Patient Tracker.
• Communicate with field staff regarding patient needs and updates.
• Ensure patients receive services according to instructions from the Admission RN or Liaison.
• Maintain positive relationships with referral sources and patient families.
• Ensure admissions are completed according to hospice contractual requirements.
• Provide support and guidance to admissions staff as needed.
• Handle other duties as assigned.

Qualifications:

• Active LVN or RNlicense required.
• Minimum 3 years of solid hospice experience
  , specifically in admissions or intake
• Outgoing personality and strong ability to work collaboratively with others.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and lead a team effectively.
• Bilingual in Spanish is preferred.
