Responsibilities
  • Provide support and give time to an allocated group of service users, and promote their recovery, supporting their return to a community environment, or helping them to maintain their place in the community.
  • Support service users to meet their identified needs and outcomes, paying attention to and respecting their values, wishes and aspirations to lead their lives as they choose, including helping them access community resources and services such as housing, benefits, leisure, education and employment.
  • Assist in the organization of clinical activity, support and facilitate group and individual work while continuing to support and deliver clinical activity as required.
  • Work independently and liaise with various third sector agencies and organizations as well as carers; establish and maintain excellent links with GP practices. Excellent skills in multidisciplinary working are essential.
Qualifications
  • The ability to speak Welsh is desirable;
    Welsh and/or English speakers are equally welcome to apply.
  • Excellent communication skills, self-motivation, proactivity and a commitment to promoting innovation for service delivery.
