What you will do:

• Conduct comprehensive assessments of adult service users, identifying their needs and developing appropriate care plans.
• Collaborate with a multidisciplinary team including Occupational Therapists and health trust colleagues to ensure holistic care delivery.
• Apply relevant legislation such as the Care Act, Mental Health Act, and Human Rights Act to safeguard service users.
• Maintain detailed records and documentation, ensuring compliance with statutory obligations and fostering transparent communication.
• Engage with service users and their families in a caring, respectful, and empathetic manner, promoting their rights and dignity.

Requirements of the Social Worker:

• Degree or equivalent in social work along with current Social Work England registration.
• Demonstrable frontline experience in UK Social Services as a qualified Social Worker.
• Strong understanding of relevant legislation and a commitment to working collaboratively with service users.
